N.E.M.T. Transportations Services 

The Non-Emergency Medical Transportation Service is a services that is paid and covered by State Insurance Medicaid or Private Insurance to current clients and clients open to the public. Transportation Service include trips to/from doctor's or medical appointments, physical therapy, medical travel, and other Insurance covered trips. There is REQUIRED documentation that is needed BEFORE services start.

NOTE: Once services start, notification of change of trip MUST be updated with agency within 24 hours of scheduled trip.  

Enrolling to utilize N.E.M.T. Transportation Services 

REQUIRED DOCUMENTATION 

Information must be submitted to agency BEFORE start of services

Do to the Non-Emergency Medical Transportation Service being funded by State Medicaid or Private Insurance, a complete Medical Record from your Primary Physician is REQUIRED to be on file while providing transportation services. Information can be faxed, mailed or present at the office in person.
